PSN Class Action Settlement Forms

The Class Action lawsuit against Sony for the 2011 security breach has been settled and the results mean that PSN account holders in North America with an account made prior to May 15, 2011 are eligible for some free stuff. The forums are pretty straightforward with PSN, SOE, Qriocity and Identify theft reimbursement each having a separate form.

When it comes to the information required for the PSN form it is basic contact information, your PSN account email and a signature. Details on the forms can be found at PSNSOESETTLEMENT.com.

People can choose one of the following options if they qualify.

– A Wallet Reimbursement for accounts that still have a balance, but have not been used since June 13, 2011. If you choose this option that means you agree to close your account in exchange for payment equal to the wallet amount.
– Participants in the “Welcome Back” program have the choice of one PS3 or PSP game from the following lists PS3: Dead Nation, inFAMOUS, LittleBigPlanet, Super Stardust HD, rain, Puppeteer, Invizimals: Lost Kingdom (PS3) and God of War HD. PSP: LittleBigPlanet, ModNation Racers, Patapon 3, Killzone Liberation, WipEout Pure and Syphon Filter Dark Mirror. Other options for “Welcome Back” participants include a choice of three themes from a list or 3 months of PlayStation Plus if they are not already subscribed to PlayStation Plus.
– For those that did not participate in the Welcome Back program you have the option to choose two games from the previous list or one game and three themes or one game and the 3 month PlayStation Plus subscription. Or take six months of PlayStation Plus if you do not already have a subscription.

The form process is straightforward, but require the forms to be mailed to Sony with August 31st being the deadline to file a claim.
